Community Health Workers

ETHC has an access to Community Health Workers who provide services to patients in their homes. They explain, reinforce, support and help people with social and medical issues in their home environment. They also help transport patients to the clinics when required. You can be benefitted by an intervention by the CHW if you:

  • cannot make it to appointments
  • are severely incapacitated
  • do not have transport
  • do not have a support person
  • require an interpreter/translation service.
Self Management Education

Self Management Education (SME) is a FREE programme in your local community for you and your family. The SME sessions aim at improving living conditions for everyone, especially for those who have been diagnosed with long term medical conditions.

Benefits of attending the SME session:

  • Learn new skills to manage your health condition.
  • Develop confidence in the way you manage your condition(s).
  • Meet others and share similar experience.
  • Learn new tips for keeping as well as possible.
  • Feel in control of your life.
  • Family supporting family.
  • Opportunity to be trained as a community leader.
  • Achieving goals and milestones.
  • Turn obstacles into milestones.

ETHC aims to empower patients to manage chronic diseases through self management education. This entails engaging patients in special awareness and community development programmes.

Discharge Care Coordinator

ETHC aims to re-engage patients with their primary healthcare provider upon discharge from secondary healthcare in order to maintain continuity of care, facilitate patient needs and reduce readmission to hospitals. If you or any of your family members have recently been discharged from a hospital, you may expect a call from our discharge care coordinator to check your wellbeing, advise you on follow ups and also give understanding on medication etc.
