Self-Management Education

Are you, or someone you care for, struggling with a health problem such as diabetes, asthma, lung disease, high blood pressure, high cholesterol, weight issues, kidney disease, heart disease, depression, anxiety, arthritis or other health conditions. 

Take the chance to feel supported by people who know how you feel.  Learn to manage your health better in the My Health, Our LIFE programme.  This self-management programme is one of the most useful and practical courses you can attend. 

Participants meet for 2.5 hours per week for six weeks.  Programmes are held in the community in community centres, marae, churches, libraries and hospitals.

The My Health, Our Life programme is based on the Chronic Disease Self-Management Program (CDMSP) which was developed at the Patient Education Centre at Stanford University.  Find out more at
